The 39-year-old has scored 10 goals in six Serie A games, but left the pitch with a probable hamstring injury in the second half.
Zlatan gave Milan the lead with a sublime header from the edge of the box and later turned in Rebic's cross after the break.
Dries Mertens pulled one back for Napoli but an on-loan Chelsea midfielder Bakayoko was sent off for the hosts two minutes later.
